To request a special prevailing wage determination, you must answer the Request for Determination and Response to Request (SF -308) and send it to the US Department of Labor, Employment Standards Administration, Wage and Hour Division, Branch Construction Wage Determinations, Washington D.C. 20210. The decision for the special prevailing wage rate is only effective for 180 days, and is only applicable to the project they are issued for.
